Care Strategies for Improving Pain Management in Elderly Patients
CHAPTER ONE
PREAMBLE TO THE STUDY
Nurses are key health professionals involved in the frontline improvement of pain management of patients, including theย elderly. The World Health Organization predicted that the proportion of people aged overย 60ย yearsย willย increaseย fromย 11%ย toย 22%ย betweenย 2000ย andย 2050.ย Alignedย withย thisย increaseย inย theย ageingย populationย isย aย greaterย demandย forย nursingย care.ย Nursesโ perceptions and challenges ofย workingย with elderly people is of global interest because of many reported negative perceptions ofย care and limited number of qualified nurses interested in the field of caring for elderlyย patients.ย Thisย studyย aimedย atย exploringย theย nursesโ perceptions and challenges ofย caringย forย elderlyย patientsย at Federal Medical Centre, Ibadan in Oyo state. The study used a mixed methodsย approach.ย Dataย wereย obtainedย fromย 148ย randomlyย andย 18ย purposivelyย sampledย nursingย staffsย through a structured interview schedule and Focus Group Discussions respectively. Binaryย Logistic Regression and content analysis of main themes were used to identify factorsย associatedย with perceptions of caringย for elderlyย patients.
CHAPTERย TWO
LITERATUREย REVIEW
ย Introduction
Literature review is an organized written presentation of what has been published on a topic byย scholars and it includes a representation of research conducted in a selected field of study. It refersย toย extensive,ย exhaustiveย andย systematicย examinationย ofย publicationsย relevantย toย theย researchย projectย (Burns and Groove, 2019). The purposes for analyzing or reviewing existing literature are toย generate research questions, to identify what is known and not known about a particular researchย problem or situation. In short literature review conveys to the reader what is currently knownย regardingย the topic of interest (Burnsย &ย Grove, 2019).
In this study literature review is intended to help to determine what is known about nursesโ perceptions and challenges ofย caringย forย elderlyย patients.ย Theย reviewedย literatureย wasย drawnย fromย variousย publicationsย accessedย fromย internetย andย otherย sources.ย Anย advancedย searchย wasย performedย usingย theย words โperceptionโ, and โelderlyโ. Only clinical research articles that stated within the objective toย explore nursesโ perceptions and challenges of caring for elderly patients were evaluated and selected for inclusionย inย theย study.
Perceptionsย ofย nursesย onย elderlyย patients
Improvement of pain management of older adults occurs in a cultural context in which the elderly in society are poorly valued. Although the concept of ageism is not fully understood. Levin and Levin (2010) suggest the term alludes to stereotypes or beliefs toward elderly people that categorize them, or attribute characteristics to them, that are not necessarily based on certain evidence. Elderly people have reported experiences of others making assumptions about their levels of illness and frailty based solely on their age, rather than on knowledge of them as individuals (Jansen and Morse, 2018). Ageist perceptions may lead to discrimination and mistreatment of elderly patients. Perceptions of elderly people are influenced by a variety of factors, including the amount of gerontological knowledge nurses possess (Baumbusch and Goldenberg, 2000), the environment in which nurses work and the workload nurses experience in diverse practice settings (McLafferty and Morrison, 2018). There are also societal views held by nurses that add to the way they perceive their elderly patients which may enhance the positive or negative treatment outcomes. Perceptions of elderly patients, at both individual and societal level can vary widely (Kite et al. 2019; Narayan 2022). Perceptions that have been reported are multi-dimensional in nature (Kite et al. 2019). In other words, ambivalent findings regarding perceptions of elderly people and ageing are indicative of the fact that most people tend to rate old age positively on some dimensions whilst rating it negatively on others (Williams et al. 2021; Gilbert and Ricketts 2022).
Whether elderly people are perceived positively or negatively is often dependent on the differentย dimensions they are being perceived on for instance physical appearance, health, and adaptabilityย amongย others.ย Further,ย itย isย likelyย thatย peopleย doย notย haveย fixedย positiveย orย fixedย negativeย perceptionsย ofย oldย age,ย butย ratherย theyย haveย differentย viewsย ofย theย manyย andย variedย featuresย ofย ageingย andย elderlyย people.

This chapter discusses the research design, the study setting, population, ethical consideration andย samplingย methodsย whichย wereย usedย toย answerย theย researchย questions.ย Theย dataย collectionย tools,ย dataย collection technique and data analysis methods, validity and reliability, pre-test was also be stated.ย Itย wasย aimedย toย guideย theย researcherย toย doย thingsย inย aย careful,ย systematicย andย logicalย way.ย Thisย studyย wasย exploringย theย nursesโ perceptions and challenges ofย caringย forย elderlyย patientsย atย At Federal Medical Centre, Ibadan.
Researchย Design
A mixed methods approach was used in this study. A mixed methods research design involves theย collectionย orย analysisย of bothย quantitativeย and/orย qualitative dataย inย aย singleย studyย in whichย theย dataย are collected concurrently or sequentially, are given a priority, and involve the integration of theย data at one or more stages in the process of research (Creswell et al., 2017). Thus quantitative andย qualitative methods were undertaken concurrently, employing a convergent parallel design. Meaning,ย theย twoย strandsย wereย independentย atย dataย collectionย andย analysisย butย mergedย atย interpretationย ofย findings.ย In this case, the qualitative results provided the content analysis that explains the quantitative findingsย (Johnsonย and Onwuegbuzie, 2018).
Theย quantitativeย approachย isย aย methodย ofย researchย usedย toย quantifyย theย problemย byย wayย ofย generatingย numericalย dataย orย dataย thatย canย beย transformedย intoย useableย statistics.ย Itย isย usedย toย quantifyย attitudes,ย opinions, behaviours, and other defined variables and generalize results from a larger sampleย population (Wyse, 2011). On the other hand, the qualitative approach is used to uncover trends inย thoughtย and opinions, and dive deeper into theย problemย (Wyse, 2011).
CHAPTERย FOUR
RESEARCHย RESULTSย
Introduction
This chapter deals with analysis and interpretation of data obtained during the study, the purpose is to address the findings unveiled in the study. The results describe information on nursesโ perceptions and challenges of caring for elderly patients, knowledge levels on aging and improvement of pain management of elderly patients, socio- demographic factors associated with perceptions of geriatric nursing care, availability of resources for care, staffing levels and management support. In this case both quantitative and qualitative findings are presented.
CHAPTER FIVE
CONCLUSION AND RECOMMENDATIONS
Theย implicationsย areย relatedย toย theย problemย underย study,ย whichย exploredย theย nursesโย perceptionsย andย knowledge of caring for elderly patients with implications for gerontological nursing training. Theย implications have their significance on different aspects of gerontological nursing which include,ย education,ย research, practiceย and administration.
Moreย emphasisย onย gerontologicalย curriculaย andย trainingย inย variousย aspectsย ofย gerontologicalย programs are strongly needed. Nurse educators should consider restructuring nursing curricula inย Nigeria so that integration of aspects related to nursing elderly people takes place early in theย program courses. The gerontological content which seems to be scant now need to increase so thatย nurses are equipped with knowledge and skill to meet the challenges of caring for elderly patients.ย Higher institutions of learning should provide such education to both in-service and direct nursesย whoย will giveย guidanceย in nursingย practice.
The improvement of pain management of the elderly patients is no longer the sole domain of specialist geriatric nurses and is anย increasing part of the clinical workload of nurses in the medical surgical wards. Care providersย require knowledge, skill and good judgment to carry out the task of caring for these patients.ย Therefore, there is need for capacity building in practicing nurses to improve their caring skill andย knowledgeย whichย will assist in reversingย theย wayย theyย perceive elderlyย patients.
Additionally,ย byย providingย theย knowledgeย andย skillsย toย nurses,ย itย willย helpย toย sensitizeย theย communityย also to dismiss the myths and beliefs about the elderly people such as pain being a normal part ofย agingย andย theย agingย process.ย Nursesย comeย fromย theseย communitiesย withย theseย perceptionsย ofย elderlyย patients which impair theย wayย theyย provideย careย toย them.
Itย shouldย beย notedย thatย thisย studyย isย limitedย toย aย particularย careย setting,ย focusingย onย theย perceptionย ofย nursing professionals, which shows the importance of further research in other realities that alsoย unveilย theย perspectiveย ofย otherย healthย professionalsย andย elderlyย usersย ofย healthย facilities.
Furthermore, it will be interesting to conduct a qualitative study to explore how the elderly peopleย feelย about theย careย theyย receive or howย theyย perceive theย nursesโย attitudeย towards them.

The practice of holistic care, which is the very essence of nursing, can be undermined due to costย pressures relating to nurse/patient ratios. One of the core management roles is to ensure staffingย levels and knowledge bases (skill mix) are allocated appropriately to ensure provision of bestย practice. Nurses report that workloads are the main factor associated with negative perceptions ofย elderly patients. It is important for managers to understand that nurses with appropriate knowledgeย and skills in gerontology which are associated with positive perceptions of elderly patients willย maximize benefits to both the patient and the institution. Therefore management should supportย gerontological nursing so that they recruit and deploy nurses who have the appropriate knowledgeย andย skill forย the core.
5.2ย Conclusion
This study explored the nursesโ perceptions and knowledge of caring for elderly patients. Theย nursesโ descriptions of the pressing issues surrounding improvement of pain management of elderly patients provide usefulย information for quality improvement activity including organizational initiatives aimed at creatingย anย enablingย environmentย forย both users.
Aย lowย awarenessย ofย ageingย andย ageingย processย wasย reflectedย inย theย dataย gatheredย fromย respondents.ย Educationalย attainment,ย exposureย toย livingย withย agedย peopleย andย workingย experiencesย helpedย nursesย recognizeย theย relationshipsย betweenย aging,ย commonย illnesses,ย andย theย requiredย care.ย Resultsย showedย that more than half of the nurses (81%) had negative perceptions of caring for elderly. This meansย theyย could not offerย goodย health careย to theย aged.
The study revealed that there was no statistical significance between nurses’ level of education,ย exposure to elderly persons, experience and the age to perceptions of caring for elderly patients.ย Therefore, the more a nurse is qualified, exposed, experienced and aged, the more likely he/sheย understood health care need for the aged and the better their perceptions about them. In terms ofย availabilityย ofย resourcesย forย improvement of pain management ofย elderlyย patientsย mostย ofย theย patientsย whoย indicatedย theย inadequacyย of resourcesย had negativeย perceptions towardsย theย improvement of pain management ofย elderlyย patients.
The biggest challenges were increased workload related to understaffing and lack of specializedย staff and infrastructure designed for the elderly people. These findings are consistent with otherย studies.
Theย elderlyย populationย inย Nigeriaย isย growing.ย Aย majorย goalย ofย nursingย educationย shouldย beย toย impactย nursesโ understanding and attitudes toward improvement of pain management of the older adult patients, as well as to influenceย their career choices toward meeting the health care needs of this growing ageing population. Anย educationalย interventionย consistingย ofย aย robustย gerontologicalย nursingย content,ย intentionalย interaction with the elderly and on-going discussions and reflections of these activities can impactย perceptions toward elderly patients in a more positive direction. Also positive feedback fromย management acknowledging nursesโ self-education and reflections about their practice could alsoย contributeย to a positiveย experienceย forย older people.
5.3ย Recommendations
Nursesโ perceptions toward older adults are important for several reasons which include, elderlyย population is increasing rapidly, elderly patients often have three or more chronic illnesses thatย requireย frequentย usesย ofย variousย healthย careย institutionsย andย perceptionsย towardย workingย withย elderlyย patientsย can influenceย career choicesย and theย qualityย ofย careย provided to elderly people.
5.3.1ย Generalย Nursingย Councilย (GNC)
Nurses who care for this population should demonstrate an interest and willingness to work in thatย field.ย Theย lowย levelย ofย nursesโย knowledgeย andย badย perceptionsย aboutย caringย forย theย agedย inย thisย studyย are associated with the lack of specific gerontological nursing training units in Nigeria. There is aย needย toย exposeย nursesย toย aย nursingย curriculumย thatย supportsย theย developmentย ofย positiveย perceptionsย and attitudes toward elderly patients. The findings of this study suggested that clinical practicumย withย elderlyย patientsย inย preย service,ย andย educationalย interventionsย withย aย richย gerontologicalย contentย can positivelyย influenceย theย perceptions of nurses towards improvement of pain management of elderlyย patients.
Therefore, The GNC should also consider to introduce a nursing specialization in gerontologicalย nursingย asย thisย willย helpย toย aidย inย consultationย andย directionย ofย careย needsย ofย elderlyย people.ย Asย such,ย in-service training through MoH should be intensified for all nurses that are interested in improvement of pain management ofย elderly patients so that elderly care can be client centered. Change of nursesโ perceptions toย acceptableย levelsย beginย withย provisionย ofย knowledgeย andย skillย thatย makeย aย differenceย fromย perspectivesย thatย areย heldย byย theย generalย publicย thatย mayย devalueย andย stereotypesย theย elderlyย people.
Further, without education in elderly patient care, the deploying body will deploy nurses withoutย appropriate knowledge and skill, which will counteract staff shortages without looking into theย plightย of the elderlyย people being attended to in healthย facilities.
5.3.2ย Governmentย throughย Federal Medical Centre, Ibadan
Theย governmentย throughย MoHย shouldย establishย geriatricย friendlyย careย institutionsย whereย nursesย canย do their practicum which in turn will begin to change the way nurses perceive caring for elderlyย patients.ย Itย willย alsoย give them aย choiceย ofย theirย work environmentย whichย willย reduceย theirย stress.
5.4ย Disseminationย ofย findings
Dissemination of research findings is the diffusion or communication of research findings byย presentation and publication to a variety of audiences, such as nurses, other health professionals,ย policyย developersย and consumers (Burns &ย Grove, 2019).
It involves the measures that would be undertaken to make known to the relevant authorities andย the study participants what the study has measured. For this study the findings will be printed andย boundย intoย reports.ย Theย resultsย ofย theย studyย willย beย disseminatedย byย submittingย aย copyย ofย theย researchย reportย eachย toย theย Universityย ofย Nigeriaย โย Schoolย Ofย Nursingย andย Medicalย Libraryย forย educatorsย andย studentsย toย useย asย reference.ย Anย executiveย summaryย willย beย sentย toย theย Federal Medical Centre, Ibadan which were the selected studyย sites, for management and staff nurses to refer to. A meeting will be arranged where presentation of research findings and recommendations will beย discussedย withย managementย ofย theย Federal Medical Centre, Ibadan.
